Medical Leave & Disability Paperwork
We understand that some procedures or medical conditions may require time away from work, school, or daily activities. Our office can assist with completing medical leave, FMLA, disability, and return-to-work paperwork related to your care.
This page provides general information about medical leave forms, processing times, and what to expect when requesting paperwork completion.
Types of Paperwork We Commonly Complete
Our office may assist with:

* The forms listed above are either provided to you by your employer or are available for download at each respective organization’s website.
- Return-to-work or work restriction forms
- School or activity excuse notes
- Surgical recovery documentation
- Discharge Summary – Presented after surgery before leaving the hospital
- After Visit Summary – After an office appointment, available upon request
Providers are only able to address patient conditions within their scope of practice and for the specific procedure they performed. Paperwork will be filled out relating to specific treatment or procedures provided by our office or our providers. Medical leave requests for other conditions must be filled out by the doctor overseeing that condition or by your primary care physician.
Before Giving Medical Leave Forms to Your Doctor
Please keep the following in mind:
- Contact your employer to determine which documents are required for your medical leave request. Every employer is different and may use company-specific documents pertaining to your medical leave.
- Sections that are filled out by the patient must be filled out prior to being sent to our office.


- All forms require at least one office visit before completion.
- Processing times may vary depending on provider availability and form complexity but are typically returned within 5-7 business days.
- Incomplete forms may delay processing.
- Completed forms are returned to the patient via in-person pickup.
- Some forms can be sent directly to your employer however a patient-signed release of information form may be required prior to sending any documentation to any non-medical office.
Recovery & Work Restrictions
Recovery timeframes vary depending on the procedure, diagnosis, and the type of work you do. Recovery instructions and work restrictions are listed on discharge summaries and in signed doctors’ notes and are not ran through any medical leave programs. At your initial post op visits, your provider will determine:
- Estimated return-to-work date
- Need for additional recovery time
- Ability to return to full duty or modified duty
Typical Post Op Restrictions include:
- No heavy lifting more than 10lbs
- Lifting or activity restrictions timeframes range from 3-6 weeks depending on the procedure.
- Limited bending or strenuous activity – Light duty working restrictions
- Temporary driving restrictions
- After discharge and while taking any prescribed pain medications
Restrictions are determined based on medical necessity and provider judgment.
Extended Leave Requests
Some patients may require extended medical leave for any number of reasons however additional records, appointments, or evaluations may be necessary before extended leave can be approved.
Please note:
- FMLA Forms may need to be recompleted to adjust for the new date. Not all forms are editable so a new blank copy may be requested by your provider.
- Our office only completes paperwork related to conditions treated by our providers
- Requests for retroactive leave documentation may not always be possible

How do I know what forms to give to my doctor?
Your employer should have someone that handles medical leave requests and can provide you with the documentation that they need to process your request.
Do I have to submit all medical leave requests prior to the procedure?
Not always, as a convenience some medical offices may fill out medical leave paperwork prior to your procedure, but medical leave requests can be submitted after the procedure.
How do I submit paperwork?
Forms can typically be sent to our office through fax, through the patient portal if available, or by dropping them off in person.
How long does paperwork take to complete?
Processing times vary and may take between 5-7 business days, but please allow adequate time for provider review and completion.
Will I need an appointment for paperwork completion?
Possibly. Some forms require an office visit or updated evaluation before they can be completed.
Can my provider extend my leave if needed?
Yes, if medically appropriate and supported by your recovery progress.
Can restrictions be adjusted when I return to work?
Possibly. Your provider may update restrictions depending on your healing and job requirements.
Does the office complete paperwork for conditions treated by other providers?
No. We can only complete paperwork related to care provided by our office and our providers.
Can paperwork be completed immediately during my appointment?
Not always. Forms may require review and provider completion outside of your visit time.
What if my employer needs additional information?
Additional documentation may require provider review and could take additional processing time.
