Billing & Payment Information

At Falls Canyon, we aim to make your billing experience as smooth as possible. Here’s what you need to know:


How it works:

Surgical billing may include separate charges from multiple providers involved in your care, such as the surgeon, hospital or surgical facility, anesthesia provider, pathology, radiology, or laboratory services.

Our office will bill your insurance for services provided by the surgeon. Any remaining balance, including copays, deductibles, coinsurance, or non-covered services, is the patient’s responsibility based on their insurance plan benefits.

Please note that insurance authorization does not guarantee full-coverage payment of all services. Patients are encouraged to contact their insurance provider prior to surgery to better understand their coverage and potential out-of-pocket costs.

Insurance Billing & Coverage

Falls Canyon Surgical Associates accepts most major insurance plans and will bill your primary and secondary insurance on your behalf. Please note that insurance verification or authorization does not guarantee full payment of all services, as final coverage determinations are made by your insurance provider.

Patients are responsible for:

  • Deductibles
  • Copays and coinsurance
  • Charges not covered by insurance
  • Non-covered or denied services

These amounts may be collected prior to your appointment or procedure.

If we are unable to verify your insurance coverage, or if coverage is uncertain, you may be asked to sign a waiver accepting financial responsibility for services rendered.

This may occur in situations such as:

  • Pending workers’ compensation claims
  • Insurance eligibility that cannot be verified
  • Patients choosing to self-pay without insurance coverage

We encourage patients to contact their insurance provider prior to their appointment or procedure to better understand their coverage, benefits, and potential out-of-pocket expenses. If you have questions regarding billing or financial policies, our office staff will be happy to assist you.


No Insurance or Paying Cash?

Patients who are uninsured or choosing to pay out-of-pocket for services will be required to complete several financial steps prior to scheduling a procedure. In most cases, payment in full is required before services are rendered unless other arrangements have been approved in advance.

Our office may provide estimates for anticipated procedure costs; however, final charges may vary depending on the services provided, facility fees, anesthesia, pathology, laboratory testing, or other related medical services.

Financing options may be available for qualified patients. All financial arrangements, payment plans, and required deposits must be reviewed and finalized prior to your procedure date.

Any remaining balances after services are completed will be billed through monthly statements. Patients are encouraged to contact our office with any billing or payment-related questions before their scheduled procedure.

For security and cash-handling purposes, our office is unable to accept cash payments made with bills larger than $20.00. We appreciate your understanding and cooperation.
